
"PYP's Working Together Agreement"
PYP forms an agreement with all Service Users which sets terms that both parties can feel comfortable to work with. We like to be very open and honest with the families we support to ensure that both party expectations are informed, understood and agreed. Additionally, the safety and welfare of a child or vulnerable adult is paramount and the sharing of information with the relevant individuals, organisations and agencies is absolutely essential.
The Data Protection Act 2018 and the UK General Data Protection Regulation (GDPR) allows practitioners to store and share information for safeguarding purposes, including sensitive and personal information which should be treated as 'special category personal data'. This includes allowing practitioners to share information, regarding 'safeguarding of children and individuals at risk', without consent if it is not possible or reasonable to gain consent.

PYP Staff agrees to:

Display professional conduct at all times.

Ensure information is kept confidential.

Share information with professionals who ‘need to know.’
​​

Work in accordance with PYP's policies and procedures.

Ensure that all one-to-one consultations and home visits are mutually agreed.

Give prior warning whenever possible if sessions have to be cancelled or re-scheduled.

Provide service users with appropriate information and resources.
​​

Provide a safe environment for parents, children & young people.
The Service User agrees to:

Inform the PYP Family & Youth Service in the event of having to cancel appointments or for non-attendance.

Give written consent for PYP to share information to professionals on a ‘need to know’ basis under the Data Protection Act 1998.

Be prepared to work in partnership with PYP for the benefit of themselves and their family.

Conduct appropriate and non-threatening behaviour at all times.
​​

Provide a safe home environment for the PYP staff.
​​

Be aware that if a member of PYP staff has any concerns regarding the safety and well being of any child or young person in the family, relevant Child Protection agencies will be notified immediately.

Sign the agreement form to confirm full acknowledgement and understanding of the ‘Child Protection’ statement above.
